What should I bring to my first visit?

As a new patient, you should bring your insurance information (insurance card), a list of any medications, vitamins, or supplements you are currently taking, a list of any allergies you may have, a brief medical history for our records, any test results, x-rays, or MRIs, and a referral number if you are a member of an insurance plan that requires pre-authorization, and your co-pay. Payment is expected at the time of service.

For your first worker's compensation or no-fault visit, you must bring your insurance carrier's name and address, the date of your injury, your employer's name and address, your worker's compensation number or no-fault policy number, and any test results, x-rays, or MRIs.

How do I make an appointment?

How do I cancel an appointment?

If you need to cancel an appointment, please provide our office with a 24-hour notice by telephone, at (716) 250-9999. If you call outside working hours, please leave a message in our voice mail system.

What types of insurance do you accept?

There are several HMOs, insurance companies, and Government program plans that we accept. Please check with your insurance carrier to verify our participation or call us at (716) 250-9999. We'll be happy to answer any of your insurance questions. Please have your insurance information handy when you contact us.

What are your physicians' office hours?

HOURS:
Monday, Wednesday, 7a.m. to 7:30p.m.
Tuesday, Thursday, 7a.m. to 4p.m.
Friday, 8a.m. to 4p.m.

What should I do if I have an emergency?

If you are in need of immediate assistance, please contact our office by telephone at (716) 250-9999 as early as possible so that we can accommodate you.

How can I get my prescription filled?

Please call our office at (716) 250-9999 before your prescription runs out. Our office does not provide prescription services on the weekends.
